The Principal Care Transformation Lead will drive meaningful improvements in our service delivery across multiple states and populations. This pivotal role will lead the continuous evolution of our care delivery models, encompassing both clinical behavioral health services and non-clinical Health-Related Social Needs (HRSN) interventions for our members. This role will work in close partnership with our Product and Design teams, who are responsible for designing and building the technology and services that our field teams deliver. This role will also partner with our field teams and customer teams to ensure our services achieve optimal outcomes for members through seamless integration of operational excellence and innovative technology solutions.
Responsibilities:
Care Delivery Analysis & Optimization:
- Conduct comprehensive analysis of current service delivery models across both clinical behavioral health and non-clinical SDOH programs
- Identify gaps, inefficiencies, and opportunities for improvement in care delivery processes across multiple states, populations and insurance products
- Develop evidence-based recommendations to enhance care quality, member experience, and operational efficiency
- Establish and monitor key performance indicators (KPIs) to measure the effectiveness of care transformation initiatives
Cross-Functional Partnership & Solution Design:
- Serve as a key operational partner to Product and Design teams in care delivery initiatives
- Lead troubleshooting and iteration efforts when existing implementations underperform, developing rapid workarounds and improvements in collaboration with field teams
- For new product development, provide critical operational input to ensure solutions are feasible, scalable, and will achieve intended care outcomes across multiple states and insurance products
- Collaborate to design operational processes and workflows that optimize technology solutions and drive successful field team adoption
- Partner with clinical teams to ensure evidence-based practices are integrated into all transformation efforts
Solution Planning & Implementation:
- Develop comprehensive transformation roadmaps with clear timelines, milestones, and success metrics
- Lead change management initiatives to ensure successful adoption of new processes and technologies
- Present transformation strategies and progress updates to senior leadership and key stakeholders
- Manage multiple concurrent projects while maintaining focus on strategic objectives
- Drive adoption of best practices and innovative approaches to address member needs holistically
Data-Driven Decision Making:
- Utilize data analytics to identify trends, patterns, and opportunities within care delivery operations
- Develop reporting frameworks to track transformation impact on member outcomes and operational efficiency
- Conduct regular analysis of program effectiveness and recommend data-driven adjustments
- Collaborate with data teams to ensure robust measurement and evaluation capabilities
Education, Experience and Skills:
- Master's degree in Healthcare Administration, Public Health, Business Administration, Operations Management, or related field; clinical degree preferred but not required
- Minimum 7-10 years of experience in healthcare operations, with at least 5 years in leadership roles
- Proven experience in care transformation, process improvement, population health, community health or healthcare delivery optimization
- Experience working closely with product development teams, technology implementation, or service design
- Experience working with behavioral health services and population health management programs with a strong background in operational problem-solving and iterative solution development
- Knowledge of healthcare quality metrics, outcome measurement, and performance improvement methodologies
- Familiarity with Lean, Six Sigma, or other process improvement methodologies; experience with change management methodologies or frameworks is a plus
- Understanding of healthcare regulatory environment, including Medicaid, Medicare, and commercial insurance requirements
- Familiarity with value-based care models and outcome-based contracting
- Knowledge of integrated care delivery models and care coordination best practices
- Strong communication and presentation skills with ability to influence stakeholders at all levels
- Ability to think operationally about design and design-thinking about operations
- Proficiency in creating and interpreting data reports. Must be able to run standard reports, customize filters, and analyze data to identify trends and inform decision-making. Experience with Sigma and Google Sheets is a plus.
- Proficiency with project management tools and software. Must be able to develop and manage complex project schedules, manage resource allocation, track project budgets, and perform risk analysis.
