This Per Diem (Contractor) RN Care Manager role is an opportunity for RNs looking to transition from a traditional bedside role and provide compassionate care to frail, elderly, and complex patients in the home.
You will collaborate with an interdisciplinary team and apply clinical triage protocols and creative problem solving to ensure that patients are getting the right care at the right time. This role is for a Registered Nurse, Care Manager (CM) Contractor, who is willing to think creatively and utilize their clinical skills in the field of Telehealth. The core objective of the RN Care Manager is to build trust and navigate patients to the right care at the right time while providing an exceptional virtual care experience through empathic listening. In addition the RN, Care Manager will provide aftercare services for our patients involving diagnostic and laboratory result follow up, escalations to our Payer Partners, patient education and other administrative clinical patient needs.
Therefore, we are seeking a motivated Registered Nurses with at least 3 years of experience for this role.
For this position an active unencumbered RN license is needed in all of our markets. *Please note, at least one of the active unencumbered RN licenses is required to start.
Key Responsibilities:
- Provide exceptional clinical virtual care, with extraordinary customer services by communicating with patients and referring to providers via live phone interactions
- Follow and apply emergent clinical triage protocols, and sound clinical judgment
- Schedule and coordinate care with our Care Coordinator team
- Follow up on diagnostic and laboratory results timely with clear and concise documentation
- Escalations to our Payer Partners and providers when appropriate
- Providing patient education
- Provide follow up calls to our patients post provider visit following protocols, which will include laboratory, diagnostic, referrals, etc.
- Work to ensure a seamless patient coordinated center experience in a care team environment where respect and listening are key to ensuring our patients have the highest quality care, and the perfect patient experience.
- Help coordinate all aspects of patient care
- Ability to leverage and document in multiple platform and systems
- Provide feedback on evolving technology tools to optimize operational efficiency and improve the patient’s experience.

- Quality chart reviews as directed

Requirements
- Registered Nurse with an active unencumbered New York State and Louisiana license
- *Please note, at least one of the active unencumbered RN licenses is required upon hire
- 3 years of registered nursing experience in a hospital, preferably Emergency Department or Urgent Care
- Ability and willingness to be licensed in all states we serve patients
- Ability and willingness to work scheduled weekends and holidays
- Required to be available to work three (8) hour shifts per month
- Required to be available to work two (2) weekend shifts every two (2) months
- Required to work one (1) Major holiday shift and (1) Minor holiday shift per calendar year. Qualified holidays are those recognized by myLaurel.
- Bachelor’s Degree in Nursing required.

Pay Rate $44/hour
